BOS 3001 Unit III Essay Brett Jones August 22, 2016 BOS 3001 Unit III Essay A utility crew was excavating and installing drain pipes at a worksite site. Two workers were
inside trench boxes, installing pipe, while a third worker operated a track hoe with an excavator
bucket appended to one of its arms. A fourth worker performed “spotter” position for the track
hoe operator, and a fifth operated a front-end loader, the track hoe operator was backfilling each
area of the trench after pipe had been installed. As the track hoe operator was swinging the track
hoe around, the bucket disengaged from the track hoe, and fell into the trench. The bucket
disembarked on the workers installing the pipe, causing death to one of them and an injury to
another, as the impact threw him from the trench box.
Corrective Actions:
1) Energetically investigate and take measures taken to avoid a recurrence.
2) Develop a comprehensive maintenance program for equipment, including inspection
logs and recordkeeping.
3) Convey safety meetings and “toolbox talks” which discuss load regulations and policies
forbidding the workers to work under track hoe operations. Provide orientation to new
hires. Discuss safety related issues in morning safety talks.
4) Adequately communicate to employee including new hires company policy and provide
written safety manual.
