Peggy Moloney Trigger 1
• You are a registered nurse working in the acute medical renal unit of a large metropolitan hospital. You are asked to admit Peggy, a 62 year old female of ATSI (Aboriginal and Torres Strait Islander) origins, who has CRF (chronic renal failure), treated with CAPD (Continuous Ambulatory Peritoneal Dialysis). Peggy is complaining of lethargy and abdominal pain, and has been febrile over the past few days. Trigger 2
• Having read the referral letter from the outpatients MO, you discover that Peggy has poorly controlled diabetes mellitus. She tells you that, although she measures her BGL 3 times per day, she eats whatever she wants, and she does not take her medications regularly. 1 10/03/2015 Trigger 3
• As part of the admission process you perform a head to toe assessment of Peggy. You notice that Peggy’s hands and feet are markedly oedematous, and she states that this has gradually come on over the past week. You also take this opportunity to drain Peggy’s CAPD and discover the effluent is cloudy in appearance. Trigger 4
• Peggy is displaying classic clinical manifestations of sepsis. Dr. Baxter examines Peggy and orders a number of diagnostic tests, including: • BLOODS – FBE, UEC, ESR, CRP, and BC
• CXR
• MC&S of effluent Trigger 5
• Resulting from Dr. Baxter’s review, Peggy has been ordered the following antibiotics: Vancomycin, Cephazolin, and Gentamycin to be administered intraperitoneally. She is also to commence slow infusion of IV fluids. 2
